SS-31 — elamipretide in the clinical literature — is a Szeto-Schiller tetrapeptide with an unusual talent: it concentrates a thousand-fold inside the inner mitochondrial membrane, where it binds cardiolipin, the phospholipid that keeps the electron-transport chain organized. By stabilizing cardiolipin, SS-31 preserves cristae architecture and reduces electron leak — attacking oxidative stress at its source rather than scavenging radicals after the fact.
That direct mechanism carried elamipretide into clinical programs for mitochondrial myopathies and keeps SS-31 central to work on heart failure, ischemia-reperfusion, aging muscle, and neurodegeneration.
